College Application Process


Transcript Release Authorization Form


By law, we are unable to release transcripts to any third party (college, agency, institution, etc.) without the express written permission of the individual whose transcript is to be sent.

  • Seniors MUST fill out and turn this form in TWO WEEKS before the earliest deadlines.
  • Within two weeks of request, Guidance Office sends High School Transcript, Counselor Recommendation, and FHS Profile

Students must send Application, Essay, Application Fee and Test Scores


Students are responsible for ensuring that Teacher Recommendations arrive at selected colleges.


Students may copy the Student Info Survey to share with teachers.


MOST colleges want two teacher recommendations (the counselor recommendation is separate). If using the common application, you must ensure the teacher’s email is correct.

Please take a very close look at the deadline list below.

It is important that you stay aware of ALL of your college and university deadlines.

In order to ensure that your materials are sent in a timely manner

(we wouldn’t want you to miss a deadline!),

you MUST turn your TRANSCRIPT RELEASE FORM, as well as

any additional forms for guidance,

AT LEAST TWO WEEKS IN ADVANCE.
