Work Permit

The state of Massachusetts requires all minors between the ages of 14 - 17 to have a work permit for each employment. 

Please download the document on this page, NOTE: This is NOT the actual Work Permit.  

Please download the form, have the employer and physician's office complete it, and return it with the minor, a copy of a birth certificate, or photo ID to The Administration Building located at 128 Washington Street.

The Administration Building Hours are: Monday- Friday 7:30am - 3:30pm.  If you have any questions about work permits, please call us at 508-979-4000 ext 110.
