Back to School Forms

Fairhaven Public Schools’ Back-to-School Forms are completed through Aspen, with the exception of Fairhaven High School, where forms are being completed on paper this year.


The Back-to-School Forms allow families to:

  • Review and update student contact information

  • Indicate whether you consent to the release of student directory information for specific groups

  • Review and agree to district policies, including the Student Handbook and Technology Acceptable Use Policy

Aspen Back-to-School Forms can be completed using a mobile phone, tablet, or computer.

Fairhaven High School: Students were given a paper Back-to-School Forms packet in homeroom to bring home to their parent/guardian. If you need another copy of the packet, please contact the high school.

Please note: Health forms are completed on paper for students at all schools and should be submitted as directed by your child’s school.

Receiving an internal error when you try to submit the forms? If you started your forms before recent updates were made, you may receive an internal error when trying to submit. If this happens, please delete the forms you previously started and begin them again. We apologize for the inconvenience!
