Back to School Forms

At the start of each school year, parents/guardians are asked to complete several forms to ensure student information is accurate and up to date. This helps our schools maintain the most current health, contact, and policy acknowledgments for students.

For the 2025–26 school year:

  • Contact verification will be collected either on paper or in Aspen, depending on your child’s school. Please refer to communication from your school for instructions on how this will be collected.
  • The remaining forms will be completed in Aspen, including:
    • Contact verification (if not paper-based at your school)
    • Policy acknowledgments (e.g., Student Handbook, Technology Acceptable Use Policy)
    • Consent to release student information
