Work Permits

Work permits are available for students ages 14-17 years old. The Permit Application must be signed by the employer, a parent/guardian, physician (if under age 16), and the applicant in order to be processed.

STEP 1

Secure Employment

Before applying for a work permit, you must first have a job offer from an employer.


STEP 2

Complete the Work Permit Application

The application can be completed electronically or in person with a paper application.

A valid email address is required from the applicant, employer, parent, and physician (if under age 16).

Students: Please use your personal email address (not your school email) when completing the forms.

How It Works

  • Upload a valid proof of age document on page 2 of the form. Accepted documents include: School ID, Birth Certificate, Driver's License/Permit, or U.S. Passport.
  • Document Upload Section
  • You will need your employer's email, parent's email, and doctor's office email (if under 16)
  • If you are not a student with FPS, a report card or letter of good standing from your current school will be requested via email
  • ReadySign will email your employer, parent/guardian, and physician (if under 16) to complete and sign their sections
  • You will receive a copy of the completed application once all parties have signed — this is not your work permit
  • FPS will send a drafted permit to you for signature via ReadySign — this is not your work permit
  • FPS will send the fully completed permit signed by an authorized agent to you via ReadySign — this is your permit
  • Print or email a copy for your employer

Before Your Appointment

  • Complete all sections of the FPS Work Permit Application (Applicant, Employer, Parent, and Physician if under 16)
  • Make an appointment with the FPS Administration Center by emailing [email protected]
  • Hours: Monday through Friday, 8 AM - 3 PM, excluding holidays

What to Bring

  • Completed application
  • Valid proof of age document (School ID, Birth Certificate, Driver's License/Permit, or US Passport)
  • If you are not a student with FPS, a report card or letter of good standing from your current school
  • You and your parent/guardian must both be present to obtain the permit

Work permits will only be issued to students who will be working in Massachusetts. If the employer is located in another state, please use that state's official website for their work permit process. Massachusetts requires all minors ages 14-17 to have a work permit for each employment — if you get a new job, a new permit is required.
